Introduction to the Hazard Log

Log new hazards, link causes and controls, and track associated risks and evidence.

Navigation

  • Add a new hazard using the purple button

  • Click on an individual part of the hazard log to quickly edit the correct part of the hazard log (e.g. click here to add another cause for HAZ-001)

Adding a New Hazard

Step 1: Start a New Hazard

  1. Click the Add New Hazard button (purple button).

  2. Enter a title and description for the hazard

  3. Use the AI hazard and harm suggestions to help brainstorm if needed.


Step 2: Add Causes

  1. Enter the cause of the hazard

  2. Options for causes:

    • Add new cause manually.

    • Link (+) to an existing cause from the cause bank, which will bring associated controls and evidence.

Top tip: you can copy (LHS icon highlighted above) or link (+, RHS icon) causes and controls from your bank.

Copy - copy the text of cause or control

Link - link the cause with all of its mapped controls and associated evidence

  • Changing a linked cause or control in one place changes it everywhere (including if you update the evidence attached in one place)

  • Deleting a lined cause or control deletes it everywhere!


Step 3: Add Controls

  1. Add existing controls or create new ones.

  2. Configure control details:

    • Control type (customisable, e.g., design, process, training, testing etc.)

    • Responsibilities (assign owners or teams)

    • Control status (customisable, e.g., Awaiting Evidence)

  3. Controls linked to a cause automatically update across the platform if edited.

  4. Add multiple controls as needed (example: phone number validation, training).


Step 4: Risk Assessment

  1. Set the Initial Risk using severity and likelihood.

    • Click the information buttons for definitions if unsure.

  2. Add any additional controls to reduce risk.

  3. Enter Residual Risk after controls are applied.

  4. Add a Justification for risk scoring (this is compulsory for a residual risk of 3 or more)


Step 5: Add Evidence

  • Evidence added to a control (e.g., staff training logs, screenshots) is linked wherever the control appears.


Step 6: Set Hazard Status

  1. Choose a Hazard Status (e.g., Draft, Awaiting Review). Statuses are fully customisable for your needs
